Please note, that One Roof only provides the screening for homelessness prevention assistance and there is limited funding available through community partner agencies.
If you are matched to an appropriate and available partner agency, you will be contacted by phone to complete a full assessment for Homelessness Prevention. Please note, completing this intake form does not guarantee you will receive a referral for financial assistance due to limited available resources.
